The PKCS11 is configured by default. It would be better to make the default as ordinary "private key in a file".
The PKCS11 configuration should be a configurable extension - state that in the documentation and in ca_openssl.cnf file.

I documented process of deconfiguring the PKCS11 crypto engine as part of #747.
When this ticket is resolved, we should rewrite those parts to "how to add PKCS11 engine"".
