Description
EAV will be shown:
- in certificate request detail (read-only when approving, or in already approved requests)
- in certificate detail
This change will enable us to:
- Add additional form attributes to certificate requests
- Search certificates by these attributes

I've done the code review (see the PR) and have only minor issues.
I haven't tested the new feature (it can't be tested without project-specific implementation at the moment) but nothing was broken when the IdM started with previously configured authorities.
